Self-Report Instrument

Instructions: There are no right or wrong answers here. Please respond honestly rather than the way you think you “should” answer. The value of this instrument depends entirely on your candor.
Before you begin, please read this
  • Who asked. Your district asked ACS to understand, in combination, how pastors across the district are doing.
  • Your answers stay between you and ACS. We keep your name with them only so you can save and return, so we can send reminders, and so you can withdraw your answers later. Nothing your Superintendent receives can be traced to you without your consent.
  • What your Superintendent receives. Combined, district-wide patterns. At the very end, you can choose to share your own results as well. That choice is yours alone, and answering honestly now does not commit you to sharing anything.
  • If you do choose to share. Your Superintendent has committed to use anything you share only to support you, never for placement, discipline, or advancement decisions. Your board, staff, and congregation never see your answers.
  • Your written answers. ACS reads them only to identify themes across the district. They are never quoted and never shared individually.
  • No follow-up. ACS will not contact you about your individual answers unless you ask us to and your district agrees.
  • Your control. You can skip any question, stop and return later on this device, and withdraw your answers at any time by emailing brandin@acceleratedchurchsolutions.com.
  • What this is not. This is not a clinical assessment, and it is not monitored in real time. If you are in crisis, call or text 988 (in the US).
  • Time. About 15 to 20 minutes.
